Spiritual Care Counselor
Location: Los Angeles, CA
Job Type: Part-time, Per Diem
Compensation: At Guaranteed, we aim to offer equitable and competitive compensation that demonstrates the value we place in our employees and our appreciation for the work they preform; the base rate for this role will likely fall between $35-$80 per diem based on task performed.
Summary
Guaranteed Hospice is seeking a passionate and empathic Spiritual Care Counselor to join our growing hospice care team in Los Angeles region. You will assist our holistic care team to guide our patients and their families through the intricacies of advanced illness, the end-of-life journey and offer bereavement assistance after the death of a loved one.
This position requires flexibility as you care for patients virtually, at home, or in-patient settings. Further, at Guaranteed, our Spiritual Care Counselors emphatically honors each patient’s spiritual beliefs and respect the unique cultures and traditions that make up our diverse community.
Job Responsibilities
Conduct a spiritual assessment within 5 days of admission to the hospice program
Assist in development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care
Provide spiritual support in accordance with on-going wishes and needs of the patient and family
Maintain and submit written clinical records including the initial evaluation, the care plan and daily notes
Evaluate the patients and family's response to, and effectiveness of, the spiritual counseling
Confirm, on a weekly basis, the scheduling of visits with the patients’ Case Managers to facilitate coordination of other staff visits
Participate in IDG and QAPI meetings and activities
Provide spiritual and bereavement support to staff as needed
Provide consultation and education to the IDG on spiritual care
Provide funeral or memorial services for patient’s as requested
Skills and Toolset You’ll Need to Bring
Ordained, commissioned or credentialed by an organized religious group
Comfortable in an interfaith setting
Ability to accept different lifestyles, cultures, beliefs and values
Must be a licensed driver with an insured automobile in good working order
The ability to communicate well, both verbally and in writing
Technical savviness: utilization of tablet, multi-line telephone, copy machine and other office equipment
Assume responsibility for self-development by continually striving to improve your Spiritual practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading
Relevant Experience
Bachelors or higher degree from an accredited seminary or school of theology
Bachelors of Divinity, Masters of Divinity or equivalent theology degree or training
Minimum of one (1) year of experience in a health care setting or with death and dying
Hospice experience is a plus
Experience providing funeral or memorial services for patients as requested
Conduct or participate in the memorial services for employees, facilities, and family members as required and requested
